What is NDIS Community Nursing Care?
Community nursing care is clinical care delivered in your own home or community by registered nurses, funded through the NDIS. It bridges the gap between everyday support work and hospital-level care — letting participants with health and complex-care needs stay safely and comfortably at home. Lift & Live coordinates this care through our registered nursing partners, working alongside our own support workers.
Community nursing covers high-intensity supports such as wound care, continence and catheter management, medication management, PEG and enteral feeding, diabetes management, and stoma care. A registered nurse can assess needs, develop a care plan, and train and delegate support workers to deliver certain high-intensity daily activities safely under clinical oversight.

Community Nursing We Provide
Wound Care & Dressing
Assessment, cleaning, and dressing of acute and chronic wounds, pressure injuries, and post-surgical sites — through our registered nursing partners.
Medication Management
Administration, monitoring, and management of complex medication regimes, including injections and webster-pack support.
Continence Support
Continence assessment, catheter care, and management plans — delivered with dignity and clinical oversight.
PEG & Enteral Feeding
Support and management of percutaneous endoscopic gastrostomy (PEG) and enteral nutrition for participants who need it.
Complex & High-Intensity Care
Clinical care for participants with high or complex health needs — including diabetes management and stoma care, coordinated with our nursing partners.
Care Plans & Training
Partner-nurse care plans and delegation/training of our support workers to deliver high-intensity daily activities safely.

Community nursing is funded through one of two parts of your NDIS plan, depending on the support:

A support worker assists with everyday tasks like personal care, meals, and community access. Community nursing is clinical care delivered by registered nurses — wound care, medication management, continence and catheter care, PEG feeding, and other high-intensity health supports. A nurse can also develop care plans and train support workers to deliver certain high-intensity daily activities safely under delegation. Lift & Live provides the support workers and coordinates the nursing through registered nursing partners.
Can community nursing support workers be trained to deliver high-intensity care?
Yes. A registered nurse (through our nursing partners) can assess, develop a care plan, and delegate or train Lift & Live support workers to safely perform certain high-intensity daily activities — such as PEG feeding or continence routines — with ongoing clinical oversight. This is a common and cost-effective model for participants who need regular high-intensity support.
What plan management types do you accept for community nursing?
Lift & Live accepts self-managed, plan-managed, and NDIA-managed (Agency-managed) participants for community nursing. As a registered NDIS provider, we can claim directly from Agency-managed plans. Rates follow the NDIS Pricing Arrangements and Price Limits with no hidden fees.
